VENDOR QUALIFICATIONS

Overview:

Vendor qualifications are a critical element of the Self-Determination Program. Medicaid Waiver requires that the FMS verify that all vendors have a valid business license and are qualified to provide the services they render in the program.

Idea
For example: Physical Therapists must have a Physical Therapy license. Respite providers must have a valid CPR/First Aid certification. Independent Facilitators must submit proof they successfully completed training per Welfare & Institutions Code. Dieticians must be licensed by the American Dietetic Association, etc.

Appendix C - Provider Specifications for Service, outlines the requirements to provide each service. Vendors must be prepared to provide Medicaid required qualifications and certifications as outlined in the waiver to the FMS to establish a vendor file and receive payment in the program.
