Lost, Stolen or Missing Checks - Affidavit

Service providers should ensure their contact and mailing information is current and updated with FMS Pay at all times. Service providers are strongly encouraged to sign up for ACH "direct deposit" payments so that payments may be sent electronically to a bank account.

In order to replace a lost or missing paper check prior to the 90 day stale dating, a service provider must complete and return the attached affidavit.
